Brilliant onward care. All staff switched on to needs. Admin staff on main desk of ward incredible at making system work for patient. Doctor excellent, cleaner understood circumstances and patient wishes - good communication.
Suggested improvements
Discharge process for palliative care patient on fast track to out-of-region needs massive improvement. So many discharge staff, not joined up. Potential to delay fast-track discharge to ridiculous extent only works because of strong intervention by ward administrators, going above and beyond. This is not right.
